Many patients have been left without insurance due to these challenging times such as frequently changing professions or not having dental plans available with their employer. To combat this we came out with our own Dental Membership Plan so we can help accommodate you during these current times. *CHILD Wellness Plan (12 & under):
* $247
Preventative & Restorative ($22.50 monthly or $247 yearly with auto renewal). Includes 100% coverage for: 1 child routine cleaning with fluoride and exam every 6 months, 1 set of bite-wing images per year, 1 Panorex image every 3 years, 1 dental emergency, vitality testing and emergency images visit every 12 months, and PA images prescribed by the dentist PLUS 20% off all treatment and products ($500 coupon for Invisalign). *See below for more information on pricing.
#Adult Wellness Plan (13 & older):
# $375 annually
Preventative & Restorative ($33.99 monthly or $375 year with auto renewal). Includes 100% coverage for: 1 adult routine cleaning with fluoride, exam and cleaning every 6 months, 1 set of bite-wing images per year with cleaning, 1 Vizilite oral abnormal early cell detection per year with cleaning, 1 Panorex image every 5 years with cleaning, 1 dental emergency visit per year with vitality testing and emergency images every 12 months, and PA images prescribed by the dentist PLUS 20% off all treatment and products ($500 coupon for Invisalign). ~Adult Periodontal Maintenance 3-4 month Cleaning Program:
~ $625 annually
Active therapy treatment every 3-4 months ($56.99 monthly or $375 yearly with auto renewal). 1 perio cleaning & scaling every 3-4 months, exam every 6 months, bite-wings every 12 months, dental exam every 6 months (during cleaning), 1 Panorex image every 5 years and 1 Vizilite oral abnormal early cell detection per year. Also included is 1 emergency visit every 12 months with any PA prescribed by the dentist, vitality testing and emergency exam. **Active Periodontal Maintenance 5-6 month Cleaning Program:
** $525 annually
Active therapy treatment every 5-6 months ($46.99 monthly or $525 annually with auto renewal). 1 perio cleaning & scaling that includes includes fluoride, exam every 6 months, bite-wings every 12 months, dental exam every 6 months, 1 Panorex image every 5 years and 1 Vizilite oral abnormal early cell detection per year. ALso included is 1 emergency visit every 12 months with any PA prescribed by the dentist, vitality testing and emergency exam. *Child Wellness Plan without auto-renewal is $347 annually. Upon turning age 13, child will be moved to the adult plan.
#Adult Wellness Plan without auto-renewal is $475 annually. This plan is not for active periodontal disease and must be approved by the treating dentist to qualify for this plan.
~Active Periodontal Maintenance 3-4 month Cleaning Program without auto-renewal is $825 annually. Patients with active Periodontal Disease will be eligible to reassess for the 5-6 month Periodontal Maintenance Cleaning Program following two years of remission of disease as approved by one of our treating dentist.
**Active Periodontal Maintenance 5-6 month Cleaning Program without auto-renewal is $725 annually. Patients with active Periodontal Disease will be eligible to reassess for the Adult Well Plan after two years of remission of disease as approved by one of our treating dentist.
